I have an old PC with and Intel Graphics media- Accelerator 900 and an 8 in 1 media reader. I believe this is an integrated graphics thingy ( techy aren't I? Lol) , so my question is am I able to install a dedicated graphics card to enable me to play better games that don't break up when trying to play or do I have to upgrade and get a new and better PC?

You don't necessarily have to get a new PC, just new hardware. Sorry, I'm as technically-dense as you seem to be, but I thought I'd share that little piece of knowledge with you! My PC is fairly old, but my brother is a technophile and can upgrade everything for me.

If you want to go with today's modern games, they all need a PCI Express graphics card to play smoothly.
Since your PC is old and has integrated graphics...I doubt you have a PCI-e slot. Or even an AGP slot.
so you might want to save up for a new PC.

You should be able to install a new video card. You will need to find out what kind of interface you have. You will need to find out if you have a PCI, AGP, or PCI Express slot. If you have onboard graphics, which you do, you probably have a PCI or AGP slot. You can buy a card like a GeForce 7600 GT which breathe new life into your computer for gaming. Make sure the card you buy has at least 256 MB of RAM on the card. If you only have a PCI slot on your computer, it probably would be better to buy a new computer than upgrade this one. PCI is an old interface and too slow by modern standards for good gaming. There are PCI video cards out there, but for a few hundred more, you can get a new system with a faster processor. You should be running at least 512 MB or RAM on your computer for gaming. 1 or 2 GB would be better.
