Like when I type it men's clothing how does it decide who is at the top?

there are literally hundreds of variables, but how often it's updated, how relevant the site is to the keywords and how many other sites link to it are very important.

when you type in a keyword and it lists sites, look at the 'similar pages' bit below it, as this will show sites linked in and give you more variety
